
The Stickney Public Health District operates three primary care clinics
providing free physician services and Health District programs
to township residents.

Residents need to obtain a Stickney Township identification card to obtain services.
Please contact any of the offices or stop by during our business hours (South
and North Clinics, 8:30a.m. to 4:00p.m. or Central Clinic 8:30 to Noon, Monday
through Friday).
A number of other health services are also available including: pregnancy testing,
well baby clinics and immunizations, communicable disease investigations and
follow-up, home health care school nursing services, vision and hearing screenings,
blood pressure and cholesterol screenings, diabetes screenings and school physicals.

Medical Clinics
The role of the Stickney Township Health Clinics is to accommodate the needs of the area residents of all ages concerning preventative care and basic health issues aimed at improving overall health care quality. Sick visits, physical exams for all ages, immunizations including flu shots for all ages, TB screenings, cholesterol, diabetic and hypertension screenings/care are encouraged. Emergency treatment or long-term critical care issues that require immediate attention cannot be handled for the safety of the client. The practice of the physicians is to supplement private MD visits and to care for those in need of routine medical services. Clients are prioritized and seen on a medical need of service basis and/or taken on a first come, first serve basis. We do our best to accommodate all clients in a timely manner.
Eligibility
Clinic services are available to any resident of the Township of Stickney. Proof of residency requires one state issued ID or driver’s license plus two forms of ID with name and address corresponding to state issued ID (no handwritten mail is accepted). Minors under the age of 18 will not be treated in the clinics unless accompanied by a parent or legal guardian. A minor’s parent/legal guardian has the right to consent to treatment for his/her child. If the parent or legal guardian cannot be present a notarized “Special Medical Power of Attorney” Form must be completed and placed in the client's chart.
Vaccine Program
Vaccines are provided to the residents of Stickney Public Health District through both in-house and state-wide programs. The U.S. Department of Health and Human Services have National Vaccine Advisory Committee’s Standards of Pediatric and Adult Immunizations Practices that must be adhered to regardless of which program provides the vaccines. Screenings
TB, cholesterol, diabetic, blood pressure, strep screenings are all available at the clinics. Minor charges exist for particular tests.
Physical Exams
All school and work physicals can be done at the clinics – including preschool, kindergarten, 5th grade, high school and college exams. These exams are free of charge and are done on a first come, first serve basis. Please bring your immunization records with you, as most school and work physical exams cannot be done without this information. Also, please help us, help you avoid the rush of last minute physicals. The clinics can do physicals all year round. Avoid the rush and come in early in the summer months.
Child Health Clinics
This clinic is held every month on the third Wednesday afternoon. It is a great opportunity to have your child immunized and seen for growth and development.
