Eligibility to receive Public Health home health nursing visits includes providing proof of residency and a physician’s order for service. No other nursing service can be utilized at the same time. Referrals are taken and an evaluation visit is completed. The nurses perform wellness checks to help residents remain in their home as long as possible with independence and dignity. These services include, but are not limited to, blood pressure monitoring, respiratory checks, safety and nutrition counseling, and medication management. No client is seen on a daily basis.
